Non-Perio Membership
$429 per year

Perio Membership
$769 per year
- 4Professional 1-hour periodontal maintenances

- ALLBitewings, intraoral X-rays and 1 panoramic X-ray needed
- 1Emergency exam
- 4Fluoride treatments
Plus 10% off all other services* — fillings, extractions, crowns, bridges and dentures included. 15% off for senior citizens.
Recommended for patients being treated for periodontal (gum) disease.

Newly diagnosed with periodontal disease?
New and existing patients who are newly diagnosed with periodontal disease qualify for 30% off the scaling and root planing treatment fee when they sign up for the Periodontal Membership Plan.

Children under 13
$275 per year, per child. Added on to a parent’s plan.
New patients
New patients can purchase either plan at their first appointment. A $150 new patient establishment fee is due in addition to the membership plan.
